Porter is hiring a Clinical Provider Engagement Partner to join the Clinical Operations Team! Candidates must reside in Michigan. This is a hybrid position with travel required. Our Nurse Practitioners go into homes across Michigan and do what a fifteen-minute office visit never can, seeing the whole person, identifying what’s been missed, and making sure that information gets to the providers who need it. What You’ll Do This role puts you in the field every day, visiting physician offices, primary care practices, and care teams throughout Michigan where Porter members are seen. You are the face of Porter to the provider community, and the voice of the provider community back to us. - Build trusted, lasting relationships with PCPs, care coordinators, office managers, and clinical leadership, becoming a known and valued presence in every practice you serve. - Help providers understand exactly how Porter’s in-home NP assessments support their patients, closing care gaps, surfacing undiagnosed conditions, and improving the quality of information available at the next visit. - Show providers the tangible impact Porter has on the patients they share, including better documented complexity, more comprehensive visits, and members who show up more prepared and more engaged. - Collaborate with provider offices to identify shared members who could benefit most from Porter’s services, and support seamless coordination between Porter’s clinical team and the practice. - Serve as a consistent, reliable point of contact, answering questions, solving problems, and making sure every provider interaction leaves them glad Porter is in their corner. - Represent provider perspectives internally, sharing real feedback that shapes how Porter’s programs continue to evolve. - Track engagement patterns and practice-level outcomes to identify where Porter’s presence is making the biggest difference and where it can grow. What You’ll Need - Active, unencumbered Michigan RN license OR current case management certification (CCM preferred) - 3+ years of clinical or case management experience; field-based, outreach, or care coordination experience is a strong plus - The kind of interpersonal presence that makes providers glad you stopped by. You communicate clearly, earn trust quickly, and follow through. - A genuine commitment to improving outcomes for complex, underserved patient populations - Strong self-organization and the ability to manage a geographic territory independently without losing the personal touch - Valid driver’s license and reliable transportation; comfort with regular local travel throughout Michigan
